Grant Application Forms

The deadline for 2008/Round 1 grant applications for the Berwick Health & Wellness Fund (BHWF) has concluded. Currently applications are being reviewed and applicants will be notified of grant award decisions via mail.

The next round will begin in July 2008. If you are interested in applying for a future BHWF grant, please read the Berwick Health & Wellness Fund Grant Guidelines. These documents provide important information about our grantmaking approach, priorities and geographic area covered by the fund. Remember, if your organization’s office is outside of the Berwick Health and Wellness service area, you may still apply if you will be serving participants/customers/clients within the service area.

In order to be eligible to receive a grant from the Foundation, an organization must hold a 501 (c) (3) non-profit status or be a governmental organization. Individuals are not eligable. All grants must benefit the residents and communities within the defined BHWF geographic boundaries.

Grant Workshop

A workshop will be offered in July 2008.
