Grant Application Forms

In 2010, the Berwick Health and Wellness Fund will be focusing more specifically on health related grant investments. Further information regarding grant opportunities will be posted as it becomes available.

The Berwick Health & Wellness Fund Grant Guidelines provide important information about our grantmaking approach, priorities and geographic area covered by the fund. Remember, if your organization's office is outside of the Berwick Health and Wellness service area, you may still apply if you will be serving participants/customers/clients within the service area.

In order to be eligible to receive a grant from the Foundation, an organization must hold a 501 (c) (3) non-profit status or be a governmental organization. All grants must benefit the residents and communities within the defined BHWF geographic boundaries.
